Accessing Your Medical Records

  • Your Rights: It is your health, and you have a right to see your medical records.
  • Two Options: There are two main ways to access your records: viewing a summary through the NHS App or requesting your full medical history.

Option 1: ‘Viewing the Highlights’ (The NHS App)

The quickest and easiest way to view your health information is through the NHS App. We strongly advise all patients to use it, as it makes managing your healthcare much easier.

  • What You Can See: The app gives you access to your coded medical record, including recent consultations, test results, and diagnoses.
  • What It Is: This is a useful summary rather than a deeply detailed full history, but it is helpful for seeing recent health information at a glance.

Option 2: ‘Reading the Full Story’ (Subject Access Request)

If you need a copy of your full, detailed medical history, you will need to make a Subject Access Request (SAR).

  • How to Apply: You must put your request in writing and sign it.
  • Requesting for Someone Else: If you are requesting records on behalf of another person, we need their written and signed consent before we can release anything.
  • Processing Time: By law, the practice has 30 days to process your request and provide your full records.
