Moving to a New Area: A Simple Handover

  • We are always sad to see a patient leave us, but if you are moving out of the area, changing your GP practice is simple.
  • Think of it as a simple handover from our care to your new practice.
  • The Most Important Rule: You do not need to do anything to de-register from our practice. This process is handled for you.

The Handover Process

Getting set up at your new practice is straightforward. All you need to do is find a new GP surgery in the area you are moving to and register with them.

  • Register: Sign up as a patient at your new local surgery. If you have children, they must also be registered with you at the new practice.
  • Automatic Handover: Your new practice will notify Primary Care Services England, and the handover process will begin automatically.

What Happens to Your Medical Records?

Once the handover begins, your medical history will follow you to your new practice.

  • Electronic Records: Your digital medical records will be transferred securely to your new practice through a fast process called GP to GP.
  • Paper Records: If you have older paper records, these will also be sent to your new practice. This part of the handover can sometimes take several months.

The process is simple. Just register with your new surgery, and they will handle the rest.
