How to Update Your Details
We want to make sure we can always reach you with important information about your health.
There are three quick and easy ways to update your details:
- Option 1: Online (The Quickest Way)
- Use the eConsult service on the surgery website.
- Complete the “Admin” form to let us know your new details.
- Option 2: By Phone
- Call the surgery and speak to our reception team, who will be happy to update your details.
- Option 3: In Writing
- Drop a letter or note into reception.
- Please include your full name, date of birth, and your new details.
What Happens Next?
Whichever method you choose, our team will update your record so we have the right information for you.
