What We Check

During your review, we carry out several important checks:

  • HbA1c Blood Test: Shows your average blood sugar over the last few months.
  • Blood Pressure: Important for heart health.
  • Foot Check: Assesses nerve function and blood supply.
  • Kidney Function: Urine test to check kidney health.

The Two-Stage Process

Your annual review is completed in two stages to ensure a meaningful discussion of your results.

Stage 1: The Data Collection (Healthcare Assistant)

  • Blood sample taken.
  • Blood pressure checked.
  • Height and weight measured.
  • Foot check performed.
  • Important: Please bring a urine sample.

Stage 2: The Review (Diabetic Nurse)

  • Discuss your results once they are available.
  • Create or update your care plan.
  • Action: You will need to book this second appointment after your first visit.

Note: If your diabetes is not well controlled, you may be invited for more frequent reviews.
